What do I need to schedule an appointment?
– Schedule an evaluation without a prescription to discover the diagnosis and prognosis, potential for rehabilitation, and what to expect with physical therapy. Previously, a prescription was needed for initial treatment, but now you can come directly for care without that script. If care extends beyond 10 business days, a referral may be needed.
– If you have seen your family physician or a specialist for your injury, you can obtain a script for physical therapy.
– Schedule a consultation or wellness assessment without a prescription at any time.

Billing Policy
AQPT is a cash pay practice. Payment is due at the time of service. Invoices are provided for optional filing with personal insurance by client. Forms of payment accepted are Visa, Mastercard, Discover, American Express, Health Savings Accounts (HSAs), Cash App or Venmo and Apple Pay. AQPT is not enrolled with Medicare, Medicaid or Worker’s Compensation.
